Trauma Patients at Risk for Hypothermia

French investigators reported trauma-associated hypothermia evolves from a combination of injury severity and potentially modifiable environmental and treatment-related factors.

Higher Revised Trauma Score (RTS) increased the odds of hypothermia by almost 70%. Intubation, lower temperature inside the transport vehicle, and lower infusion fluid temperature also increased the risk.

The risk declined significantly if patients were clothed and did not have a head injury, as reported online in Critical Care.

“Optimal patient management could contribute to limit heat loss or even to increase patient temperature when required,” Frederic Lapostolle, MD, of Hopital Avicenne in Bobigny, France, and co-authors wrote. “Undressing patients should be avoided. Mobile unit temperature and fluid infusion temperature were independently associated with hypothermia. They should be routinely measured.”

In addition, the group pointed out that mobile intensive care units should be equipped with “a warming system “to allow ‘body temperature’ fluid infusion, especially in severely injured patients.”

Trauma patients often have hypothermia on arrival to a hospital, and even moderate hypothermia can adversely affect prognosis. Causes of hypothermia remain unclear, and available data have come largely from hospital registries and retrospective studies.

To identify potential causes of hypothermia in trauma patients, Lapostolle and colleagues performed a prospective, multicenter, observational study involving  eight French hospitals and a regional emergency medical service (EMS).

EMS units in France are equipped for intensive care requirements, including anesthetics, catecholamines, laboratory facilities, and ultrasound, the authors noted. An EMS team consists of an emergency physician, critical care nurse, and a driver who has training in basic life support.

The study included all trauma patients older than 18 who received prehospital care from an EMS team and who were transported to a hospital by an EMS mobile unit.

Investigators recorded the following:

  • Patients’ demographic and morphologic information
  • Nature and circumstances of the trauma incident
  • Patient’s condition upon arrival of the EMS team
  • Environmental conditions (wind, rain, ground temperature)
  • Clinical characteristics (wound features, vital signs, RTS, oxygen saturation, tympanic temperature)

Investigators also recorded pertinent aspects of each patient’s care until arrival at the hospital.

The primary endpoint was hypothermia upon arrival at the hospital and the authors defined hypothermia as a body temperature of less than 95°F (35°C).

Data analysis comprised 448 patients, of whom 64 (14%) had a body temperature less than 35°C when they arrived at the hospital. Patients with and without hypothermia did not differ significantly with respect to demographic or morphologic traits.

In a univariate analysis, numerous clinical, environmental, and treatment-related factors were associated with the odds of hypothermia. Multivariate analysis revealed six factors that independently predicted lack of hypothermia on arrival at the hospital:

  • No intubation: odds ratio 4.23, 95% CI 1.61 to 11.02 (P=0.003)
  • Revised Trauma Score: OR 1.68, 95% CI 1.29 to 2.20 (P=0.0001)
  • EMS unit temperature on arrival at site of trauma: OR 1.20, 95% CI 1.04 to 1.38 (P=0.01)
  • Infusion fluid temperature (>21°C or 69.8°F): OR 1.17, 95% 1.05 to 1.30 (P=0.003)
  • Patient remaining clothed: OR 0.40, 95% CI 0.18 to 0.90 (P=0.03)
  • Absence of head injury: OR 0.36, 95% CI 0.16 to 0.83, (P=0.01)

“Routine temperature measurements should help improve the care of trauma victims,” the authors concluded. “When providing early care, EMS should always look for hypothermia. The severity of injury, mobile unit temperature, and medical interventions were risk factors associated with hypothermia on the victim’s arrival at hospital. Mobile unit and infusion fluid temperature should be measured and increased if necessary. Unclothing of patients [often done to examine the patient] should be avoided.”

The authors noted several limitations, including the fact that the results cannot be generalized to all trauma victims because the most severe cases (failure of onsite resuscitation) and least severe cases were not transported in the medically equipped mobile unit.

Also, they did not include outcomes in this study but pointed out that “the relationship between hypothermia and death is now well established.”

They called for a prospective study to assess the effect of warming on patient morbidity and mortality.

Neglecting Your Children: Shaping their Future by Not Participating in It

Neglecting your children can go beyond not buckling seat belts or forgetting to put food in the house. In fact, more damage can be created within your children from simply not being there when they need you. The lack of participation within their lives will make them figure out life on their own, which may not be the best of methods. Without interactions from you, the only role models they may have are those from TV or friends who don’t really have a clue themselves. How can you become a part of their life without making it intrusive?

1. Conversations – Don’t become a parent that only talks to their children when something is wrong. Children are people too and having a conversation about anything that is on their mind can help build a bond between you. The more you can engage a child in conversation, the more likely he or she will be to come to you in regards to various situations later in life. Lecturing is not the same as having a conversation and can easily be ignored. A conversation requires two-way communication that engages the child which can help him or her retain wisdom you may have.

2. School Functions – Every child wants to please his or her parent. Plays, musicals, sports and other school functions provide the child with a method in order to help gain the approval of his or her parents. By not attending, your child could grow to believe that other situations are more important to you. You may not have to attend every single one, but making a big deal of the ones you are able to attend can help fill your child with pride and accomplishment.

3. Online Activities – Involve yourself in the online activities with your child. Participate in gaming while providing the wisdom of safely using the Internet. Engage your child in a conversation about using social media and how to adapt to cyber-bullies. Explain how certain behaviors are inappropriate and become involved in their Internet usage.

4. Sharing – Knowledge is the most powerful tool you can give any child for any situation. However, the delivery of the knowledge needs to be in a form that they can relate to. When you try to lecture your children regarding certain behaviors, eyes could easily become glossed over. As stated above, you have more of an impact in the child’s life if the conversation was more formal and open between you. Don’t be afraid to share your knowledge of specific circumstances that fit situations. If your children understand that even you make mistakes, it could make them more open and trusting that you are just like them.

5. Family Activities – Schedule time for family activities. Allow your children to have a say in some of these. Although you may think certain things are fun, they may not. Even if they pick an activity that you don’t know, think of it as an experience to live as they do.

You can neglect your children without putting them in harm’s way. Your children need attention and guidance from the most important role model in their lives. Parents need to do more than just set rules and enforce them. They need to help shape the path that sons and daughters will walk as they mature.

Heat Exhaustion: More Dangerous than Many May Realize

Every summer, there are many people who suffer from varying forms of heat exhaustion. While the symptoms may not be life threatening, continued progression of heat can lead to heat stroke – which can cause brain damage or other heat related injury to internal organs. This doesn’t mean that you risk heat stroke if you break a sweat working outside in the yard. However, heat exhaustion could be a precursor to a more serious problem if not dealt with.

1. Dizziness – A number of bad situations can be caused by getting dizzy from too much heat. If your automobile doesn’t have air conditioning and it is over 100-degrees Fahrenheit outside, you could easily turn that vehicle into a speeding death trap. If you ever feel dizzy, it is time to sit down and stop whatever it is you are doing.

2. Fainting – Another potentially hazardous symptom of heat exhaustion is fainting. Overheating yourself could cause you to faint regardless of what you’re doing. There is usually very little warning before you simply drop to the ground.

3. Nausea – Sometimes being nauseated from heat exhaustion could be misdiagnosed as a simply stomach flu. However, stomach flu usually doesn’t go away by dropping your body temperature. If it is hot outside and you feel nauseous, take a cool bath and see if it goes away.

4. Headaches – Suffering from headaches is a common symptom of heat exhaustion. Usually, you can deal with the headaches by drinking more water and saturating yourself. It is always better to drink water when it is hot as opposed to other beverages that need to be metabolized for useful materials within your body.

5. Muscle Cramps – Although cramping may not be a sickening or otherwise debilitating condition, some can hit so hard that you cringe in agony for a few minutes. This is another signifier that adding more water to your diet could be greatly beneficial.

6. Rapid Heartbeat – An increased pulse could be a dangerous instance for those suffering from heat exhaustion. If you suffer from high blood pressure as it is, subjecting yourself to such extreme conditions could become fatal or cause severe damage to your heart. As soon as you feel your pulse begin to escalate, it is time to cool yourself down.

7. Urine Sample – If your urine is a dark yellow color, this is a signifier of dehydration. If this is discovered in the morning, it could be a warning that any physical activity today could be dangerous unless you drink more water.

Treatment: Essentially, treating heat exhaustion means to cool your body by any means necessary. This means removing any tight clothing, taking a cool bath, ice wrapped in towels, air conditioning, and above all, drink more water. Keepingyourself hydrated during the hottest months of the year can stave off heat exhaustion unless the heat is simply unbearable.

Too many people have tried to “tough it out” while working manual labor jobs in the scorching heat. Some of us were simply not built physiologically to sustain specific temperature variations. This has nothing to do with being macho, but it has to do with genetics. Pushing yourself too hard in the Sun can cause a life altering complication that could become fatal. Take care of your bodies this summer and work smarter, not harder.
