Elder Abuse

February 27, 2013

Elder abuse is an umbrella term referring to any knowing, intentional, or negligent act by a caregiver or any other person that causes harm or a serious risk of harm to an older adult. Elder abuse includes physical abuse, neglect, sexual abuse, emotional or psychological abuse, financial or material exploitation or abandonment. Elder abuse can happen within the family. It can also happen in settings such as hospitals or nursing homes or in the community. Elder abuse is a serious problem in this country, affecting as many as 2 million elderly persons. Elder abuse occurs among all racial, ethnic and economic groups. Healthy, as well as frail, aging adults may be victimized. Although elderly men may be victims, the profile of the older adult at greatest risk for abuse is a disabled woman, older than 75 years of age, who is physically, socially or financially dependent on others. Perpetrators may be acquaintances, sons, daughters, grandchildren or others. Most often, physical and emotional abuse stems from stressful caregiving situations. Abuse is also associated with a family history of violence, alcohol or substance problems and emotional or cognitive dysfunction of the abused and/or perpetrator. All elderly patients should be screened for abuse in privacy. An abuser may be reluctant to leave the patient’s side or become angry, overprotective or defensive. Questions about abuse are less threatening when asked, matter-of-factly, in the context of a social history. To ease into a more in-depth screening for abuse, you might say, “Just to make sure you’re okay, we ask all patients questions related to their safety.” A full inspection of the elder’s body should be performed. After assessing and screening the patient, the elder’s response, as well as any suspicious assessment findings should be documented in detail. Being alert for patterns of abuse, as well as paying attention to the patient and caregiver’s interactions, are essential when caring for elderly patients. Additionally, reporting suspected elder abuse is the law in all 50 states. Healthcare providers must know the system for reporting suspected abuse in their state. Although elder abuse occurs to a lesser extent in healthcare facilities, maltreatment in institutions also needs to be policed and violators reported. Dunlap, MAEd, RN, M. (2008). Assessment of elderly abuse. Grown Up, volume 13 (3).

High Speed Internet Service

February 25, 2013

Secretary: Item #3 for the business meeting is “Updating Our Internet Service” for our healthcare facility.

Chairman: What did you find out Member #2?

Member #2: As you all know, we had tornado wind damage 2 weeks ago. Prior to this, we had planned to update our Internet Service. The best company that can change our Dial-Up Internet to DSL is Copper.net/.

Member #1: What other companies did you consider?

Member #2: All of the other companies I looked at could not bring Internet Service to us. If they worked at it and did bring us Internet Service, it would be Dial-Up.

Member #3: We have to remember that we are in the rural part of Mississippi. It is a well-known fact that high speed Internet Service is not widely available in CA and MS.

Member #4: You are correct that Lumberton, MS is considered rural with our 2,000 plus population.

Member #2: Copper.net/ stated that we are in the best zip code 39455 for our town where DSL service may be found. I will add that Copper.net/ is a member of the Better Business Bureau.

Member #5: I move that we take a vote.

Member #1: I second the move.

Chairman: Those in favor of using Copper.net/ for DSL Internet Service, say yes.

Member: #1 – #5: Yes.

Chairman: It is unanimous.

Driving under the influence of carisoprodol

February 24, 2013

Many people frequently drive cars, operate heavy machinery, while under the influence of prescription medications, even though it is clearly labeled they must not do so. Depending on the injury being treated, perhaps it would be beneficial to use a natural treatment for muscle relaxation rather than rely on such a potent drug such as carisoprodol.

When there is a desire to use a natural treatment, people still need to consult with their healthcare provider. This includes those treatments taken by mouth and applied to the skin/muscles. For example, the natural herb valerian or valerian root may cause drowsiness. Another example, a hot pack may lead to discomfort while driving or operating heavy machinery.

People also need to read the labels for Over The Counter medicine and therapies. Some of these will tell the consumer not to drive or operate heavy machinery. If the consumer has trouble understanding the labels, the consumer should consult the pharmacist or the professional on staff. Many products list a toll-free number and website that can be contacted. There are also some general public consumer hotlines and websites that can be contacted.

Quitting Smoking, review

February 19, 2013

There is a group recruiting people to join a smoking cessation study. This is a good thing. The site describes the people who may have an interest in joining the study. These include the young and old adults. Details are listed about the trial. There is a newsletter with posts dated from 2011 to 2012. The latest post is “TRENDS IN SMOKING: WOMEN SMOKING IN DEVELOPING COUNTRIES.” There is a page that provides more details about who benefits from the study. And there is a page to find out more information. It indicates it will attempt to locate a participating location within 50 miles of your desired spot.

One thing that is not desirable with this website, is the popup free newsletter signup. Sometimes a new page will open instead of the desired page. The new page is apparently a free newsletter signup. Both of these are very distracting.

The pictures and map are very good quality and pleasing to see.

Quitting smoking support is provided by this website. I am glad to see this. I am reluctant to offer this website to most of my clients due to the popups and webpage switching. I would rather see a link for signing up for a newsletter.

Diabetes and Police Force Article

February 19, 2013

Recently it has come to the attention of the police force and diabetic specialists that greater attention is needed in the area of caring for diabetic patients who are being held in police custody. There have incidences where a detainee has suffered the affects of a diabetic attack while the condition went unnoticed or misinterpreted by the law officers. It is easy to mistake the symptoms of a hypo or hyperglycemic state as the side effects of drug or alcohol use. For the safety of both parties (derangement and violence are not uncommon in metabolic crisis and death can even occur in the patient) some police forces have decided to take the precaution of providing diabetic training and medical supplies for their stations. (Management of Diabetes in Police Custody: a liaison initiative between diabetic specialist service and the police force. Wright, et al. Practical Diabetes International, March 2008, vol. 25.)

Psychiatric Patients

February 19, 2013

Forensic nursing involves the application of science and nursing in legal situations. Psychiatric nurses care for patients with mental illnesses. Forensic psychiatric nurses are employed in institutions such as a juvenile homes or prisons where a majority of the residents require treatment for mental conditions. Many of theses patients also struggle with the dual problem of mental illness and drug abuse. Patients who abuse drugs present with the additional complications of addiction, dangerous behavior, withdrawal symptoms, etc. It is interesting to consider that people who abuse drugs frequently also have a mental disease, which may or may not be diagnosed. For this reason all healthcare providers should be at least basically prepared to identify and care for patients with such conditions as they may not always be found under the strict care of a mental health facility and could present for treatment in emergency rooms and doctors’ offices.
