My brain is very surprised at the results. For many years “Help Wanted” signs could be seen on doors and windows of businesses. Perhaps there are differences between Internet, newspaper, and physical sign terms. “Occupations” appear to be a general term about open positions and general descriptions. “Employment” should lead toward the specifics of open positions.

Trayvon Martin’s manner of death

Trayvon Martin is the 17-year-old youth who was killed by George Zimmerman (a 28-year-old man). This happened on February 26, 2012.

There is a Death Investigation proceeding. From a forensic nurse perspective, I want to know if this is a criminal or noncriminal homicide. It appears the local law enforcement agency is considering it a pending noncriminal homicide while an investigation is ongoing. This does not mean that it cannot be changed to a criminal homicide later.

With the U.S. Justice Department looking into this case, it will take longer to find a conclusion. Also with emotions very strong across the country, a conclusion will not be quick.

Medication Safety, comment

I appreciate your article Medication Safety. My work as a forensic nurse I see as helping people. I am not set out to put people in jail or prove them wrong. Our consulting firm is about helping institutions and their employees do their jobs without major errors. I too have witnessed people cry when they realize a lethal mistake they have made. If I can help them prevent future mistake, I am happy. If I can help others avoid mistakes, then we have done our service well.

Documentation

Documentation is critically important in obtaining any type of encounter in the ED, but yesterday I had the opportunity to apply what I have learned in the coursework to the fullest. My patient was a fifteen month old who was accompanied by her mother. The child was brought in for head injury. Her behavior was appropriate for a toddler, smiling and interacting with both mom, and me and very active. She was cooperative with the triage. Mom was concerned about the abrasion over her left eye and ecchymotic area under that eye which she stated had happened 2 days prior when the child had fallen off the couch and hit her head on the coffee table. She denied LOC, vomiting or lethargy and stated her activity level had been the usual state. On further exam, I noted a three cm ecchymotic area over the left sphenoid which could be consistent with the fall, but it troubled me that she also had ecchymosis lateral to the right eye, petechial contusions to the inner auricle of the right ear but none on the pinna and also noted pinpoint abrasions to the right occiput. This did not seem consistent with the story. I asked mom about these and she stated that “She is just learning to walk and falls a lot.” A fifteen month old is usually walking fairly well, and running unless developmentally delayed, which she seemed to be bright and happy, so this troubled me. However, since triage was busy and it is not the function of triage to document all findings but just identify them, I marked her as urgent and notated my suspicions of abuse due to the inconsistency. Later, when I got off triage, I took over this patient and got to interact with the police investigator and county child protective worker who had been called. The doctor felt that the findings were consistent with abuse and a report was filed with immediate action taken. I measured all the areas and took photographs and logged them as well as documenting all the individuals and actions we had taken. The child was CT’d which thankfully was negative. The mother had admitted that she had left the child alone with her significant other on a number of occasions. The investigator felt he had enough evidence to make an arrest. The child was released to the custody of the mother and her parents. This story had a happy outcome but I still feel troubled by it. How can a person abuse a sweet innocent toddler? How can the mother allow this? Aside from that, she was so benign about the whole matter and practically lied to me about it in triage. As a mother and patient advocate, this boils my blood. CDC and Healthy Aging

The Centers for Disease Control and Prevention (CDC) has a very nice website. Their mottos are saving lives, protecting people, and saving money through prevention.

The CDC.gov has a subpage titled Healthy Aging. It is packed with information (too much for my seniors). I see Health Aging Topics with a dozen links to topics. I see Interactive Data Tools with its 3 links. There is a Listserv link. There is also contact information.

I am especially interested in subtopics related to senior citizens. I clicked on the “More” link for Enhancing Use of Clinical Preventive Services Among Older Adults: Closing the Gap. It took me to the CDC Features subpage. I see “Clinical Preventive Services for Older Adults.” From here, I see a link to “Injury, Violence, and Safety.” It is not very clear and user-friendly the way the sub-paging is laid out.

Now, I am at CDC Features and subpage Injury, Violence, and Safety. I realize this new page is a general page and not the older adults page. However, I see topics of interest such as Older Drivers (2 separate editions), Elder Abuse (2 separate editions), and Fall Risks for Older Adults.

Even with the CDC site being very large, it can still be made user-friendly, especially for the seniors.

Senior citizens may also be interested in Medicare Supplemental Insurance.

Alzheimer’s disease

Patients diagnosed with Alzheimer’s disease usually experience symptoms in their mid to late 60s. These symptoms usually include memory loss and confusion, developing to severe dementia. Because of their confusion, Alzheimer’s patients in nursing homes sometimes wander from their rooms or beds and fall and injure themselves, accidents that can sometimes be fatal.
Forensic nursing may be required in such situations. Nursing homes are accountable for the accidents that occur in their facilities. The extent of the Alzheimer’s disease in the patient sometimes needs to be determined for legal reasons. This is accomplished by examining the brain and the presence of neuritic plaques and neurofibrillary tangles. (Lehne, Richard A. Pharmacology for Nursing Care, 6th ed.)

Medication Errors

Medication errors occur when a patient receives the wrong drug or dose or is given a drug at the wrong time or by the wrong route. If a medication is administered without consideration for possible side effects or drug interactions there may be dangerous results for the patient. Elderly patients are at high risk for medication errors because they frequently take many medications at the same time which can result in drug interactions or dosage mistakes. Problems can also occur if patients do not report all of the over-the-counter drugs or natural products that they may be taking. Even drugs that are “natural” and considered safe may still interact with other prescribed medications and have harmful effects.

Medication Errors

It is impossible to stress the importance of going through all of the rights of drug administration. A nurse must be alert and ready at all times. A good example is epinephrine. Epinephrine is can be administered by several routes: IV, IM, subQ, intracardiac, intraspinal, inhalation, and topical (157). The strength of epinephrine depends on it route of administration. The same concentration would not be administered for IV and IM. IV administration would less concentrated than that of IM and most other routes. Always be careful in looking at the orders and when drawing up the medication make sure it is for the correct route. If one is not careful they could potentially cause a fatal reaction that should have been avoided if only they had used the drug rights of administration. Whether one has been a nurse for 6 months or 16 years, it does not allow leeway for one to skip a step.

Wife beating/Wife battering

Violence against women has been a long-standing problem and unfortunately been incorporated as an accepted practice in some cultures. The two terms are not interchangeable but are two separate concepts of unacceptable behavior ranging from physical, sexual and emotional abuse as well as emotional deprivation against a spouse or significant other. Wife beating, sometimes thought to be more frequent, is physical aggression that occurs in 85% of industrialized countries according to the text. On the other hand, wife battering is repeated assault with the intent of the abuser to maintain complete control of the female. This situation escalates in severity and frequency and could lead to homicide. Control issues or cultural beliefs may be some of the factors that lead to these situations. For example, the Hispanic culture has the very strong machismo trait of male domination and the Native Americans are also the domineering males although, interestingly, they are a matriarchal society in that the Clan Mothers have a voice in many important issues, at least in the Iroquois Nation here in New York State with whom I have many close friends. Both beating and battering cause physical and mental problems, and should raise a flag of suspicion for the forensic trained nurse. Both require assessment and intervention to prevent the female from becoming a statistic. According to the text Forensic Nursing by Lynch, 49 studies from 36 countries showed the highest prevalence (up to 52%) of wife beating/wife battering among the Arabs, Palestinians, in Nicaragua, Korea and New Zealand. “Other relatively high prevalence areas included Antigua, Barbados, Egypt, Ethiopia, Bangladesh, India, Kenya, Nigeria, Papua New Guinea, Uganda, Turkey and [surprisingly] the UK” Lynch and Duvall (p. 54, 2006). The US, Switzerland, Canada, Philippines and South Africa were much less at 20% to 30%. Norway, Paraguay, Puerto Rico, Cambodia, South Africa and Zimbabwe were only between 10 and 20%. Being a native Norwegian, I can speak to this, since Scandinavia is largely a matriarchal society, based on its Viking heritage. I suppose this stems from the days when the women were left on their own to raise their large families while the men went to sea and therefore had to become independent!
