Alzheimer’s disease

Patients diagnosed with Alzheimer’s disease usually experience symptoms in their mid to late 60s. These symptoms usually include memory loss and confusion, developing to severe dementia. Because of their confusion, Alzheimer’s patients in nursing homes sometimes wander from their rooms or beds and fall and injure themselves, accidents that can sometimes be fatal.
Forensic nursing may be required in such situations. Nursing homes are accountable for the accidents that occur in their facilities. The extent of the Alzheimer’s disease in the patient sometimes needs to be determined for legal reasons. I recently graduated from Law School, and absolutely could not be more excited. My entire family is over the moon, too. Not only am I the first person in my extended family to go to Law School, I’m actually the first person in my immediate family to graduate college. They couldn’t be prouder of me. I can’t go anywhere with my parents without them telling someone their daughter is an attorney. It’s pretty adorable. Know what’s not adorable? The student loans I racked up attending Law School. I am so fortunate that I received a great job with a local firm right after graduation, but the money I’m making is not even going to make a dent in my student loans. Medication Errors

Medication errors occur when a patient receives the wrong drug or dose or is given a drug at the wrong time or by the wrong route. If a medication is administered without consideration for possible side effects or drug interactions there may be dangerous results for the patient. Elderly patients are at high risk for medication errors because they frequently take many medications at the same time which can result in drug interactions or dosage mistakes. Problems can also occur if patients do not report all of the over-the-counter drugs or natural products that they may be taking. Even drugs that are “natural” and considered safe may still interact with other prescribed medications and have harmful effects.

Medication Errors

It is impossible to stress the importance of going through all of the rights of drug administration. A nurse must be alert and ready at all times. A good example is epinephrine. Epinephrine is can be administered by several routes: IV, IM, subQ, intracardiac, intraspinal, inhalation, and topical (157). The strength of epinephrine depends on it route of administration. The same concentration would not be administered for IV and IM. IV administration would less concentrated than that of IM and most other routes. Always be careful in looking at the orders and when drawing up the medication make sure it is for the correct route. If one is not careful they could potentially cause a fatal reaction that should have been avoided if only they had used the drug rights of administration. Whether one has been a nurse for 6 months or 16 years, it does not allow leeway for one to skip a step.

Wife beating/Wife battering

Violence against women has been a long-standing problem and unfortunately been incorporated as an accepted practice in some cultures. The two terms are not interchangeable but are two separate concepts of unacceptable behavior ranging from physical, sexual and emotional abuse as well as emotional deprivation against a spouse or significant other. Wife beating, sometimes thought to be more frequent, is physical aggression that occurs in 85% of industrialized countries according to the text. On the other hand, wife battering is repeated assault with the intent of the abuser to maintain complete control of the female. This situation escalates in severity and frequency and could lead to homicide. Control issues or cultural beliefs may be some of the factors that lead to these situations. For example, the Hispanic culture has the very strong machismo trait of male domination and the Native Americans are also the domineering males although, interestingly, they are a matriarchal society in that the Clan Mothers have a voice in many important issues, at least in the Iroquois Nation here in New York State with whom I have many close friends. Both beating and battering cause physical and mental problems, and should raise a flag of suspicion for the forensic trained nurse. Both require assessment and intervention to prevent the female from becoming a statistic. According to the text Forensic Nursing by Lynch, 49 studies from 36 countries showed the highest prevalence (up to 52%) of wife beating/wife battering among the Arabs, Palestinians, in Nicaragua, Korea and New Zealand. “Other relatively high prevalence areas included Antigua, Barbados, Egypt, Ethiopia, Bangladesh, India, Kenya, Nigeria, Papua New Guinea, Uganda, Turkey and [surprisingly] the UK” Lynch and Duvall (p. 54, 2006). The US, Switzerland, Canada, Philippines and South Africa were much less at 20% to 30%. Norway, Paraguay, Puerto Rico, Cambodia, South Africa and Zimbabwe were only between 10 and 20%. Being a native Norwegian, I can speak to this, since Scandinavia is largely a matriarchal society, based on its Viking heritage. I suppose this stems from the days when the women were left on their own to raise their large families while the men went to sea and therefore had to become independent! Save Our Children, Save Our Society

It has been common belief and practice that the only way to rid society of crime was the execution of offenders. This practice, however, has not had a significant impact in deterring criminal activity. In fact, violent crime seems to be even more proliferative. Studies have shown that the vast majority of violent offenders and serial killers were abused physically and psychologically as children. We must not treat crime as an isolated criminal justice issue, but rather a national public health problem. In order to formulate and institute a better approach to ending violence, attention must be turned to the importance of parenting and socialization of our children. Positive parenting should include the teaching of morals, motives, and social roles. It is also a necessity that we instruct children about their culture and how to live within it. The vital role of the forensic nurse is to utilize the components of nursing, science, and the law collaboratively in order to aid all victims in their medical treatment/recovery, provide proper/pertinent documentation of records for testimony in the apprehension of criminals, and promote programs for crime prevention/public safety. Toxic Workplace, comment

This comment may be a little off-topic.

It was so painful for me as my wife Lil and I watched the events of September 11, 2001 on television. Seeing a plane hit the World Trade Center (WTC) North Tower at 8:46, then a second plane hit the South Tower at 9:02. Shortly, we saw the firefighters and other first responders courageously going into the buildings hoping to extinguish the fires, but it was impossible to foresee what followed.

Then we watched in shock as nearly a dozen people were jumping from the upper floors to their deaths.

We felt profound horror at 9:59 as the South Tower cascaded in freefall into its own footprint, and then 29 minutes later when the North Tower came down in the same impossible freefall way.

The new forensic evidence which is being released today by Architects and Engineers for 9/11 Truth demonstrates the presence of controlled demolition materiel in the World Trade Center buildings One and Two.

Just one week after September 11, Environmental Protection Agency (EPA) Administrator Christine Todd Whitman declared “I am glad to reassure the people of New York and Washington DC that their air is safe to breathe and their water is safe to drink” and that we “. . . need not be concerned about environmental issues as [we return to [our] homes and workplaces”.

Yet to this day, at least 900 first responders have since died as a result of the effects of toxic “dust” from the buildings and the some 3,000 human remains that enveloped lower Manhattan and which Mayor Rudolph Giuliani declared “We must clear the rubble”. This “rubble” in fact constituted evidence from a massive crime scene, but was hauled away, first to Long Island, and then was eventually placed on barges and shipped to China.

One thing I know is that the official government story of those events, as well as what took place that day at the Pentagon, is just that, a story. This story is not the truth, but far from it.

I was born on October 12, 1932. I am announcing today that I will be consuming only liquids beginning Sunday until my eightieth birthday in 2012 and until the real truth of what truly happened on that day emerges and is publicly known.

In an article by Dr. Holly Lucille, N.D., environmental toxins are brought up as a common health issue. Every day, workers all over the world are exposed to health threatening substances in plastics, pesticides, car exhaust, soaps, emulsifiers, health and beauty aids, household cleaning products and foods.

Your workplace exposure could increase the level of toxic substances, especially in older buildings which have not been properly maintained. Newer building built with toxic chemicals and that are constructed so tightly that insufficient fresh air is introduced into the building with poor ventilation, leaving allergens and irritants trapped inside. New installations, carpet, wall coverings, paint or construction can all heighten problems with VOCs. Toxic mold thriving in cool, damp, dark places behind walls and under carpeting. Construction products such as finishes, heavy duty cleaners, paints, thinners, dry cleaning fluids, some copiers and printers, some glues and adhesives, markers, and photo solutions are among some of the common office products that emit harmful volatile organic compounds (VOC).

This can be further amplified in atrium style buildings or buildings with interior water features, if inadequate care results in mold issues. Dander, molds, and dust mites are carried by animals and people into and throughout buildings. Occupational allergies are common. Many articles are written on “Sick Building Syndrome” and “Occupational Asthma.” where people have symptoms including watery eyes, runny nose, headaches, dizziness, nausea and tightening sensation in the chest. Most of these symptoms are that they have a distinct pattern of getting worse at work and improving on vacations or weekends.

Malfunctioning, inappropriate, or inefficient use of heating devices can produce irritating pollutants such as carbon monoxide, nitrogen dioxide and sulfur dioxide at harmful levels. Formaldehyde exposure is widespread and found in resins in finishes, plywood, paneling, fiberboard and particleboard, and in some backings and adhesives for carpets. New installations, carpet, wall coverings, paint or construction can all heighten problems with VOCs.

Bring a hepa or ionic style air purifier to work, keep area free of dust and clutter, increase your intake of pure water and increase your intake of antioxidants can aid your body in its ability to fight the toxins. In addition, speak with your company OSHA representative if the problem is too bad.

SANE program

I just wanted to share that I’m excited to be a part of an evolving area of nursing at the facility I work at. We are researching the start of a sexual assault nurse examiner (SANE) program. The way we process sexual assaults presently is so detrimental to the victim and the evidence. None of nurses have been trained in forensic nursing so we always have to take the kit, and directions into the room with us and read and work as we go. It’s a one on one situation which takes a nurse out of commission making her feel rushed to get the process over with, which is not fair to the victim. The SANE nurse is trained in the collection of forensic evidence, the exam and needed follow up referrals; they’re also trained in the process of judiciary proceedings. It will be a great asset to have a nurse on call to spend the needed time with the patient as well. This training will also help in the collection of data for possible domestic abuse cases.

Poor Care Delivery

Medicare is certainly sending healthcare providers a message. They will not pay for poor care delivery. An example of this is that they will no longer pay if a patient falls or develops a bed sore. I applaud this. We should not add to our patient’s demise by adding additional disease processes to the one the came in with. While it may be true that someone with low immunity could develop a nosocomial infection, it can not be tolerated that we leave them unsafe with the ability to fall and break a hip. My hospital has developed techs that sit “one on one” with demented or confused patients. This has drastically reduced the falls.

Dehydration

When we moved to Utah, the air was so much drier than it was back home. I noticed that my skin had gotten really dry and I constantly had a headache. It was going to be about three months before I could get to see a doctor in Salt Lake, so I decided to go http://www.satelliteinternetbroadband.com/hughesnet/Utah/ and get satellite internet so that I could do some research from home. There are some really great websites that address common health complaints. You do have to be careful that you find a legitimate site like WebMD, or else you can run into horror stories on the internet and cause yourself a lot of anxiety. I ended up searching all of my symptoms. In regards to the headaches, someone suggested on a message board that I may be dehydrated everyday. When you sweat in dry heat, you don’t even really realize that you are hot and sweating because it dries up so quickly. They also suggested that my headaches could be caused by living at a higher altitude. I started drinking two huge bottles of water everyday and low and behold my daily headaches went away. I am still going to see the doctor, but will be sure to check into my complaints next time I can’t make it to the doctor for three months!

Some of our clients and fellow workers have asked for confidential referrals regarding treatment. They include first responders, forensic nurses, forensic doctors, and other forensic personnel. Non-health professionals and worker also request for referrals. Issues include depression, alcohol addiction, drug addiction, anxiety, grief reaction, and post traumatic stress syndrome. This issue list goes on and on.

One such facility that answers our needs is the Birmingham rehab center. They provide service not only to adults, but also to adolescences. Family members are an integral part of the adolescence’s success in a drug abuse rehab center. Family members are also important in the non-drug abuse treatment programs.

They have programs for professionals such as physicians and lawyers. Their services are confidential. They have an intensive outpatient program, intervention services, and emergency consultations. The emergency consultations are part of their community program and are offered without charge.

Drug abuse is the number two reason for referrals we would make to this center. As a drug abuse rehab center, this is excellent. You may ask what is the number one reason for referrals. Our number one reason would be alcohol abuse. This center is also excellent with alcohol abuse treatment.

This facility boast accreditation by the Joint Commission on Accreditation of Health Care Organizations.

Insomnia and the elderly

Not only is insomnia an issue for the healthy older adult, it can be even worse for the older adult with dementia as they frequently get their sleep/wake cycles confused. Families often call or present at the office requesting sleeping aids. These sleeping aids for healthy elders pose their own risks, but for a dementia patient these risks are increased. Sleep aids often cause confusion, which can lead to increased wander. With increased wandering, there is an increased potential for falls, especially if there are stairs in the home. The likelihood of a patient actually leaving the safety of the home is increased, with the very real possibility of becoming lost. Another issue associated with increased nighttime confusion is often frantic phone calls, most often to adult children/siblings, which can disrupt other households. These phone calls also are known to go to emergency response personnel requiring them to respond to the home for a non-emergency situation. For all these reasons, the use of sedative/hypnotics as sleep aids in the elderly, especially those with dementia, should usually be avoided at all costs. Other pharmacologic interventions, which have been found to be useful with fewer side effects, are trazadone or melatonin. These medications usually work, but the patient can develop a tolerance, requiring higher doses. These medications are not addictive, but higher doses increase side effects. Non- pharmacologic interventions include herbal teas or gentle massage, much as you would do to an infant’s back. The overall goal, of course is return the patient to the appropriate sleep/wake cycle. No one intervention is always effective, and several mat need to be tried. The use of sedative/hypnotics should always be used as an absolute last resort and only for very short periods of time.

A Form of Elder Abuse that goes unnoticed

Understanding Elder Abuse is a topic that is now a hospital competency for healthcare workers. As a nurse in the ED there are situations where the abuse is evident and other times more subtle and seen through the interactions between the elderly patient and his/her family. What about those families that bring grandma or grandpa to the ED right before a Holiday with a list of symptoms that guarantee an admission? Only for us, the staff to hear the family tell us they are going out of town for the Holidays and won’t be available for X number of days. It is a situation that, when assessed, is a form of Elder Abuse. It happens more often with those elderly patient’s who are not able to verbalize well, have dementia/Alzheimer’s or cannot speak for themselves. With an overhaul of the healthcare system there will have to be an overhaul of the social services in this country because without it there will be no way to provide safe, effective and complete care to the most vulnerable in our population.

New Scrubs Needed

Last week, I was involved in a forensic autopsy. My nursing scrubs became very stained even though I tried to prevent it from happening. On this particular day, I did not have an extra set of scrubs in my locker. Boy, was I glad to get home, change my clothes, and set out a fresh pair of scrubs for the subsequent day. Uh-oh I said to myself. All of my scrubs were in the laundry basket to be washed. So I placed all of my used scrubs into the washing machine. Hemophilia

Hemophilia A is the most common hereditary disease associated with bleeding. It is caused be a decrease in amount or activity of factor VIII. Factor VIII is an important protein involved in the coagulation process, mainly in the intrinsic pathway. Hemophiliacs have a tendency to bruise easily, and have massive hemorrhage after trauma or surgical procedures. Recurrent bleeding in joints also occurs in people with this disease. When hemophiliacs bleed, they do not bleed faster than normal, they bleed longer because the coagulation process does not function properly. This condition can lead to death and may not be diagnosed until an autopsy is performed by medical personnel, such as forensic nurses.

Ever since getting my masters degree I feel like a weight has been lifted in my life. Now I’m in a new city with new people and everything is extremely exciting. I guess this is a new stage in my life and I should reach out and embrace everything that comes my way. The only thing I need to do now is find a stable job and start making some money. Hopefully, I can tread water for a couple months on the money that I’ve saved from graduation presents and some money my father gave me. I recently moved into a new apartment uptown and besides the noise at night I love it! I feel like it’s a purchase that I needed to make and I feel more secure knowing that my house is more adequately protected. Now that I know my place is protected I can focus all my energy around looking for a job.

I was planning to move to California because marijuana is legal in this state. Today, I read that marijuana is legal in 17 states (including the District of Columbia). So now I have 16 more reasons to live. I have severe arthritis and pain from a hit-and-run automobile accident. My doctors have prescribed a lot of medications. Some of them work and other do not work. The ones that do work last for a few weeks or months. My family is packing my bags right now. In a few days, marijuana will be giving me some needed relief.

Bullet-proof vests (especially for our president in dangerous situations)

Hello. I am seeking information on several things that I do not know how to talk about, or ask about, so I will give it my best shot.Pardon me if it sounds crude to ask, but I don’t have the technical language.

My questions involve the similarities and differences in the manner in which child victims of violence sexual abuse, or homicide are examined, treated, autopsied, etc.

Is there a resource where a layperson can find data?

My particular questions ( again I apologize if I sound crude)

1) when little boys are killed, does anyone attempt to seek evidence of saliva in their genital regions?

2) is there a rape kit for boys, and is that kit different then that for girls?

3) when a little boy dies at the hands of a female caretaker, is a rape kit performed, or saliva samples collected from the genitals? Whatif a masle is a suspect? What if a little girl is killed?

4) when little boys are killed by women or girls, is the autopsy procedure the same, or different than for little girls?

5) if little boys are sexually abused, does the doctor or nurse ask them gender specific questions, i.e. ‘did your dad,…’ which are followed up by ‘ did your mom…?’ In other words is the forensic exam egalitarian across the board?

This is just the tip of an iceberg of my questions. Can you answer these, and point me to online resources for self study?
